What Is a Vasectomy?

A vasectomy is a minor surgical procedure that helps men permanently prevent pregnancy. The procedure blocks sperm from reaching the semen during ejaculation. Vasectomy offers a simpler and safer alternative to the more invasive “female sterilization” or tubal ligation and eliminates the need for harmful female hormones used for birth control.

How Does a Vasectomy Work?

The testes are responsible for sperm production. Each testicle has a tube called the “vas deferens” that carries the sperm to the semen. Vasectomy involves cutting and closing the vas deferens through an opening in the scrotal sac. This prevents the sperm from reaching the semen, thus preventing pregnancy.

How Does Nitrous Sedation Work and What Are the Benefits?

Nitrous is a non-allergenic, non-irritating gas that affects the central nervous system. It is sometimes referred to as “laughing gas” and can provide excellent pain control and reduce anxiety during the procedure. It is one of the safest forms of sedation available, and patients report very few, if any, side effects. Nitrous is not intended to put you to sleep, so you will be able to hear, give feedback, and respond to any requests or directions Dr. Zargaroff may have.

Dr. Zargaroff will ask you to breathe normally, and if you’re like most patients, you’ll begin to feel the nitrous oxide take effect within 3-5 minutes. Following treatment, the effects of this gas are completely reversed within 5 minutes simply by breathing normally.

The No-Needle Technique

Dr. Sherwin Zargaroff specializes in no-needle, no-scalpel, single-site vasectomies. During this procedure, nothing except the anesthesia solution initially penetrates the skin. This is accomplished using a state-of-the-art MadaJet anesthetic device.

The MadaJet device uses pneumatic pressure or “burst of air” to infuse the skin with an anesthetic solution. This step allows the vasectomy to be needle-free, with better pain control.

No-Scalpel, Single “Keyhole” Technique

After anesthesia is confirmed by the patient, Dr. Zargaroff will locate the vas deferens under the scrotal skin. He can then use a single midline “keyhole” site, if the anatomy permits, to reach both vas deferens. This will leave the patient with one small procedure site instead of two and thus reduce post-procedure pain.

Dr. Zargaroff will then use a micro-dissector rather than a scalpel to specifically puncture the skin around the vas deferens. This technique minimizes the overall incision size to several millimeters and further reduces post-procedure pain.

Each vas deferens is then divided — the upper portions are sealed with heat energy, and the lower portions are kept open (open-ended technique). The two ends of the vas are then separated, and a fascial interposition is performed by Dr. Zargaroff to prevent them from reconnecting. The procedure site is then closed, and the procedure is concluded.

Unlike conventional vasectomies, this procedure doesn’t involve excision or the removal of tissues, making it safer. The procedure is performed in our state-of-the-art procedure suite and usually concludes within 20-30 minutes.

Why Open-Ended Vasectomies Matter

Dr. Zargaroff performs his minimally invasive vasectomy in Miami using an open-ended technique to minimize the possibility of post-vasectomy pain (PVP) syndrome, which can rarely be seen with conventional closed-ended vasectomies. PVP syndrome can be debilitating and is thought to involve a pressure gradient in the closed vas deferens from the buildup and congestion of sperm. An open-ended vasectomy technique is performed without closing the lower end of the divided vas deferens to prevent this pressure gradient from forming. The sperm is thus not blocked and free to be naturally reabsorbed back into the testicle, thus reducing the potential for this complication.
